How to Apply
Complete Application
Complete Online Application, must be typed and can’t be handwritten. Use the website provided to complete the application and then print and sign. Original signature is required, in Black Pen.
Valid U.S. Passport
- You must submit your current undamaged passport and it must have one blank visa page.
- The passport must be signed and valid for six months from the date of departure.
Visa Photos
- Passport photo services available at our offices.
- Must be 2″ x 2″ on a white background.
- Photo must not be older than one month.
Proof of Travel
Applicant must submit flight and hotel reservations.
Confirmed Hotel Reservations
Copy of confirmed hotel reservations. Note, If staying with friends or family, a letter of invitation from the host in Bolivia, can be used instead of the hotel reservation. The letter should be addressed to the Embassy of Bolivia, Visa Section and should state the relationship to the applicant, the dates of the visit, and the address and phone number where you will be staying.
Proof of Finances
Applicant must submit a copy of their most recent monthly bank statement.
The statement must clearly show the applicant’s name as the account holder, the balances of the accounts, the date of the statement, and the complete transaction record for the month.
Medical/Health Requirement
Applicant must submit original copy of an International Certificate of Vaccination for Yellow Fever.

Business Cover Letter
Applicant must submit a Business Cover Letter (BCL) from a US company. A BCL is an original document on company letterhead signed by a company official. It must provide the dates, duration and purpose of the trip. It should also include contact details (name and address) of an individual at the host company. Close the letter by accepting moral and financial responsibility for the applicant.
Letter of Invitation
Applicant must submit a letter of invitation from the host company detailing the relationship to the applicant and the purpose of travel. In addition, the letter must indicate the length of stay, place of stay, and contact information.

- You can include a prepaid return air bill in which case you will not be charged.
