To qualify, your most recent U.S. Passport:
- Is undamaged and can be submitted with your application.
- Was issued when you were age 16 or older.
- Was issued within the last 15 years.
- Was issued in your current nameĀ or you can legally document.
If you have a previously issued passport card, please include it.
How to Apply
- Do notĀ sign the application until instructed to do so.
- You mustĀ provide a Social Security number, if you have been issued one, in accordance withĀ 22 U.S.C. 2714aĀ andĀ 22 C.F.R 51.60(f)
You must presentĀ oneĀ of the following identification documents (IDs).
- Fully-valid U.S. driver’s license
- Fully-valid, undamaged U.S. passport (may be expired)
- Certificate ofĀ Naturalization or Citizenship
- Government employee ID (city, county, state or federal)
- U.S. military ID or military dependent ID
- Valid foreign passport
- Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)
- You must submit your current undamaged passport.
- The expired passport will be returned to you along with the new passport.
- If you have a copy of your flight itinerary or ticket, please include it.
- If you do not have a travel itinerary or a ticket,Ā itās okay.
- Passport photo services available at our offices.
- Must be 2ā³ x 2ā³ on a white background.
- Photo must not be older than one month.
In the event that you changed your name due to marriage or divorce, you will need to bring in a certified copy of your marriage certificate or a court decree that shows your name change.

- You can include a prepaid return air bill in which case you will not be charged.
