- Submit Official or Diplomatic Passport, must be signed, have at least six months remaining and one blank visa page
- 2 Visa Application Forms completed and signed
- 2 Recent Passport-Type Photographs
- Letter from sending Government Department or Agency, addressed to ” Embassy of Barbados, Visa Section, Washington D. C. ” detailing the applicant’s name and occupation, Purpose of the trip, Dates of the trip and contact person in Barbados
