How a name change works

In order to change the name on your passport you must have an original court ordered name change, original marriage certificate or divorce decree.

You must renew your passport in order to change your name. If your passport was issued less than a year from the date of issue and you need your name changed or corrected in your passport, you will only need to pay the expedite fee to the US State Department to have it corrected.

Bring the document and the current book to Suite 203 and Sameday Passport & Visa prepares the application, checks the document against the passport, and files it by mail.

Which route your passport takes

  • Issued less than a year ago: a correction, government fee $60.00.
  • Issued within the last 15 years: a name-change update filed and expedited by Sameday Passport & Visa, government fee $213.36.
  • Issued more than 15 years ago: you will file a new passport application instead.
  • Photocopies and notarized copies of the name-change document are not accepted.

How to apply

What to have ready before you come in.

Complete the online application
Do not sign the application until instructed to do so. You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).

Current US passport
You must submit your current undamaged passport. The expired passport will be returned to you along with the new passport.
Original court documents
You must provide the original or certified copy of the name change document from the court — an original court ordered name change, original marriage certificate, or divorce decree. Photocopies are not accepted.
Flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay — tell Sameday Passport & Visa when you come in.
Passport photos
Passport photo services are available at the Chicago office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.

Applying by mail

Name changes are filed by mail, so a mail-in order works well. If you are processing your application with Sameday Passport & Visa by mail, fill out two Letters of Authorization and the service order form and send them along with the documents to Suite 203.

Government expedite fees must be paid by cashier's check or money order, payable to “US Department of State”. Emergency replacement and limited validity replacement passports depend upon application type.

Adding it up for a name change

A passport issued in the last year is the $60.00 government fee plus the service fee. Anything older is $213.36 plus the service fee. There is no execution fee on a name change, because it is filed by mail rather than at an acceptance facility.

  • Include a prepaid return air bill and you will not be charged for shipping.
  • Provide a FedEx account number and the Chicago office will generate the air bill, with a $5 fee added to your work order.
  • Or collect the new passport in person at Suite 203.
  • Return shipping is an optional service. Sameday Passport & Visa only returns passports using FedEx.

What do I need to change the name on my US passport in Chicago?
You must have an original court ordered name change, original marriage certificate, or divorce decree — you cannot change the name on your passport without one of these documents. You must also submit your current undamaged US passport, which is returned to you, a completed DS-5504 application form, unsigned until instructed, an original or certified copy of your name change document, your Social Security number, and a passport photo.
