Cambodia – Passport and Visa required

Visa Requirements for U.S. Citizens

TOURIST

• Valid and signed U. S. Passport – must be valid at least six(6) months from the date of entry in Cambodia and must have blank(2) visa page for visa sticker and entry stamps.  
• One Visa Application Form completed and signed. The application form must have original signature.
• Copy of Passport bio-page (passport name & photo information) – Black & white copy is accepted.
• Computer generated flight itinerary
• One Passport Type Photograph (size 2 x 2) – recently taken at least six(6) months
• Hotel Confirmation
• Sameday Passport Service Order Form

BUSINESS
