US Passport Name Change Requirements

Prerequisites for Passport Name Change

(if the below criteria are not met, please select a different passport service above)
  • Applicant has US passport is in his or her possession, AND;
  • Applicant has legally changed their name since most recent passport was issued AND;
  • Applicant’s passport was issued within the past 15 years AND;
  • Applicant applicant was 16 years or older when passport was issued.

1 Current Passport / Card

Applicant must submit their current valid, or recently expired, passport (and/or passport card if applicable) in order to renew. The old Passport will be returned marked “Cancelled” by the US State Department

2. Passport Application (DS-82)

Applicant must submit one completed Passport Renewal Application Form (DS-82) which is “barcoded” – handwritten applications are not accepted for expedited services.

3. Passport Photos – (This service is available at our all locations)

Applicant must submit two identical, 2″x2″, color, passport-style photographs.

4. Name Change Documents

Applicant must submit an original or certified copy of the marriage certificate (not a license) or a court decree ordering a name change or an adoption. The document must show the official seal of the issuing authority. All official documents will be returned.

6. Letter of Authorization – Please call us at : (512) 905-2001

Please complete and print out two copies of Authorization Letter. This letter will authorize our company to submit and obtain the new passport on your behalf. Please call us for a Letter of Authorization due to limited Slots available for passports Submission or walk in our office from 9am -3pm

7. Processing Time & Fees

A. U.S. Government Fee:

Make one check or money order payable to “U.S. Department of State” (this is the government processing fee).

  • Passport Book: $209.53 (expedited)
  • Passport Book and Passport Card: $239.53 (expedited)
  • Regular passport book(9-13 weeks process time) $130
  • Regular passport book and card (9-13 weeks process time) $160.

Note: Temporary or starter check is not acceptable.

B. Processing Time & Service Fee:

  • 3-5 business days processing
  • 7-10 business days processing
  • 15 business days processing
  • 20 business days processing

8. Order Form
